Welcome to Multicultural Psychological Issues about Mental Health

Each Client has A Unique Mental Mechanism

THE PORTFOLIO OF A CLIENT IN THE PRESENT SELF-CENTERED WORLD

As human beings have limitations to diagnose depending on the factual data provided in a session, if a person does not trust or disclose the inner self, complexes and has not gone through testing and regular face-to-face interaction, even an angel cannot find the facts without the will of God. How can a therapist know why a person is following him for some unknown needs, goals and wishes but all at once after a year of deep communication, drop on a note to point out faults in a clinician?

It is a psychological challenge to handle clients with predisposition, irrational belief system, stored aggression, sadistic tendencies, self-projection, suicidal ideation, low self-esteem and self-torturing tendencies, guilt-oriented and self-punishing obsessive thoughts, ritualistic trends as solution to environmental problems, unrealistic goals with impractical objectives, short-cut techniques for deep-rooted complexes, judgments about people or objects, unnecessary concern and uncertainty in people and world affairs, over worrying without action about future, perceiving other ugly without realizing and resolving jealousy or aggression, and who are moody in company.

In fact, human thinking is an interwoven chain of thoughts depending on the storage of memory with moods. Under the law of association, words generate a chain of repressed thoughts. If a client has lost control on his cognitive functioning power, he is at the mercy pf intense thoughts with linked feelings. For instance, when someone is in love with a beloved, he focuses and feels according to the inner thinking pattern (personal world) and is lost in memory of the beloved residing miles away. He even becomes a poet or artist who perceives spring as gloomy autumn. He has elaborated thoughts for that person, mostly loses appetite, sleep and routine activities of a peaceful life. Mostly, his built-in computer with an abnormal frequency feels everything negatively, cries at sad songs under desperation and suicidal ideation. Most of the poetry, film songs, novels, fine arts, etc., are the creation of such creative negative thinking as the sadist feels a soothing state in this process.

Clinically, the gap between now (thinking) and then (full action to relies) is called a time of anxiety and psychic confusion. A plan is a theory or fantasies (pains or anxieties) till activated repeatedly in a person to turn it into practical achievement for mental satisfaction and relief in sufferings. The client has to come out of the self “cocoon” and listen and feel, by cutting doubts or discussions (defense or arguments, delays or priorities) of I or your territories. Rather, above all one should activate basis only WE as a built-in unit within the personal spaces, committed to sharing a pure and intact reservoir of feelings. Personally, one should get involved in charging and recharging (psychologically called a rapport prior to establishing deep hypnotic state, i.e., love, respect, belongingness, security, deep trust) each one for the bond for mutual fruitful results. One should feel proud over the bond and enriching it with maximum time in participating in one-to-one level interaction (meetings) for a “durable, peaceful, soothing life.”

Only for clinical output a client should, within boundaries and professional ethic, take the therapist as an ideal person whom he can blindly trust that he will not be betrayed, as the therapist is equipped with knowledge of how to cut rigidity by reaching the same mental level. The altered state of mind or client’s internal wish to be hypnotized for a peaceful solution is dependent on the same level of friendship and frequency. So, the client should not be confused in this process and waste time in games rather for his own mental health, either fully endorse it for practices or come out openly to reject it without any reservation. It is necessary for both peace and activating toward the set goals for an integrated IDEAL UNIT. One cannot build castles in the air. Mutual level of needs and sincerity demands full support with each other’s good intensions.

At times, client try to develop a mental bond through media communication: it is hard to explain or understand others’ delicate feelings through e-mail as the words do not show the non-verbal feelings or facial expressions. So, the sole solution left is one-on-one interaction by psychic frequency and same mental caliber with individual respect and exchange of feeling under trust and confidentiality. It is suggested to avoid such practices as they are harmful for the sensitive, confused clients. There is great possibility of misinterpreting the signals when they are not properly conveyed. So, it creates hundreds of complication or confusions, rather than deepening the already created bond. If the client does not understand the therapist, the client may be right on his ends as he never disclosed himself or whatever the therapist got in one meeting was incomplete.

During the process, prior to starting the therapeutic sessions, some clients play roles to trick or check the therapist’s knowledge and professional skills, to grab technical knowledge for their own practices among the friends. Some are overloaded with information but when derailed in anxiety, become serious clients. A few without resolving their inner self, try to be self-clinicians and turn critical over self-assessment techniques. Others enter the therapy seriously but hold valuable information and when cross-questioned, look for another favorable therapist like shopping seriously but returning home with empty hands. Some force therapists to follow their pleas and not reach the hidden guilt feeling; they often turn aggressive to an insulting level but reappear after weeks to restart from point zero; with unstable moods, they restart without changing the inner self on an old road with an old car without overhauling the engine or combustion, etc. It is worth consideration that he is under flow of pain and suffering and trapped in chains of thought, so he keeps on shifting to one point. Under such circumstances, it is hard to issue step by step to reach the destination. A clever client is controlled by an intriguing unconscious force which never likes to lose grip for a normal mental functioning system. It is a big professional challenge to handle such a crisis in which overt and covert variables are ruling a mentally sick person. Certain clients who have fanciful thinking patterns become reluctant to take responsibilities or follow an activity chart by making lame excuses and putting burden on the other’s shoulders. The same obsessive client looks for lacuna to justify that they are helpless to follow a designed activity chart by indulging in the conversation of ifs and buts. They wish to remain in denial to see the ground reality and face hardships of the cultural pressures as a defense mechanism. Prompting with short goals makes it easy for them to come out of the chain of ideation.

Some are referred by friends or other physicians, brought by a family member, visit after reading an article, very few look for self-analysis for innovation, others rush for emergency session under the spell of manic depression.

Present Era Contains A Strange abnormal world of varying clients

WHERE THERE IS A WILL, THERE IS A WAY

To handle the present complicated clients in a capitalist society, the therapist no doubt needs a strong personality with various tools, therapies and insight of human nature; otherwise, there are possibilities to be derailed under the psychic pressure of a client’s emotional dams. Some will try to fool you by role playing, some will try to overload your weak areas, some will offend you in blaming, some will take you as an ideal or a fountain of knowledge or a messiah, some will look up to you as a pier

In a strange abnormal world of varying clients, the therapist has to keep his identity while digging through a land full of powerful smells; the therapist is the link between the fanciful world and factual realities, a lost identity and family bonds, etc. This profession is a tough task in the materialistic race for honor as self-projection, ruled with extreme and unsympathetic people, and hypocrite patterns to trick others for personal gains. Mostly, a client’s background is affected by the mal-treatment by family and friends, cultural and religious shocks. Some are insulted, rejected, assaulted, sexually abused, humiliated, deprived at the sensitive age when they severely needed security, love, appreciation and sincere care to improve as a strong, integrated individual. Naturally, they have physically grown up but remain childish in nature. Whenever they confront environmental challenges or setbacks, they regress to a deprived, lonesome, helpless, hopeless, defeated, aimless creature on this earth. They cry, scream, yell and weep like a hunted child. They desire that the therapist may apply some miracles to come out of their old net of mental complexes.

Definitely, the professional duty of a therapist is to care for such emotionally deprived and socially ignored class at the cost of his own self by opening the hidden locks of their complexes. Under such conditions, the therapist has to play a key role to provide some balanced psychological sentiments to kindle the light. The psychologist’s task is not only to resolve his current issues with an integrated personality in the clinic but prepare for handling the personal and social challenges within his environment and culture with responsibility and logic. He needs to be trained in meeting the spiritual and psychological needs and to provide feedback in love, warmth, security, respect, and appreciation boost to other people at a balanced level.

The client should be equipped to live like an individual with self-respect, hope, independence and maturity to tackle the social issues with mutual respect and care for each other in an integrated whole, like a “figure and ground relationship”.

For the therapists, no doubt it is a tedious job in a traditional society with dual cultures and religious practices, where they are labelled Freudian, sex-oriented healers whereas quacks, piers/fakirs, and mullahs are applauded by a propogandist group in the same society. Although cases of fraud, sexual assault, and money laundering are common in the ignorant of rural areas, the crooks make them go away by their influence in the court procedures.

Clinically, a therapist must keep his identity and be objective in analysis as he should have empathy for the client and not sympathy; for example, he should not be crying with the client in a bout of grief or depression as it will strengthen and reinforce the client’s thoughts. Moreover, the resolution of the client’s imagery is not a practical application of thoughts rather treatment lies on the principle of imagery resolution within the world of imagination. No doubt, the most difficult treatment is of those clients who are rigid in nature and live in active fantasy but it is my professional insight that if they were born a normal human being, it is curable with professional skill, insight and the committed faith to help the client toward well-planned and goal-directed tasks. Systematic desensitization may be a reliable strategy for such clients in daily life.

The therapist’s job is to create insight to follow the middle path as the sole reality is to avoid learning from extremes and to correct one-self for a normal life. In fact, extremity in any form is abnormality and causes pains in long praying, hoarding, secluding. It is a loss of road from the middle path when often people bar themselves from enjoying their surrounding happiness. The client should be made aware not to indulge in idealism as it has no edge or destination.

At the final stage of treatment, the rehabilitation process is a very hard and complicated process for life adjustment. It is observed that clinically adjustment disorder turns out to be a common cause in all the human resolution in an environment. The cultural variable must be given proper attention in individual life adjustment. In this regard, besides well-planed therapy, activity charts, clarification of misconceptions, and handling the new outcomes of cultural and religious issues, the therapist involves the concerned relative, friends or colleagues like a facilitator in the situation. The family, marital unit, children, elders, office colleagues, etc., play a key role in the process of rehabilitation. The communication gap and lack of effective methods of interaction become a key issue at this stage of adjustment. Religious, recreational, physical and social involvement make a life meaningful. In the western psychological rehabilitation process, the social worker plays a significant role to prepare the other concerned members. The cooperation makes it easy for the client to establish himself quickly for ever-lasting adjustment when it is facilitated by other friends, family, social and religious people.

The client may get insight by the therapist to enjoy the moment as life is short but to make it meaningful. The purpose of life is not to live for instinctive needs only as an animal. He has to fix short goals to achieve and share to enjoy others’ feelings and good wishes in his community and with significant others. Living alone is just denial of reality and love begets love, one should enjoy what has been blessed with as all will stay here when departing this world.
